Step 1. Go to Google Forms and click “Create form”

Step 2. Give a title for this form and add a description if you need it.

Step 3. Open an option and tap on the “Multiple choice” box. Then, change it to “File upload”. After that, read the notice given on the screen and click “Continue”.

Step 5. After all the settings, click the “Publish” button in the upper-right corner of this page. Here you can adjust the permissions for editor and responder. Then, click “Publish” again in the pop-up window.
